- Address, Place & Date of Birth, Nationality, email address, Social
- Security No., Telephone No)
- Marital Status
- Education
- Particulars of Mother & Spouse (Name, Surname, Other Names,
- Maiden Name (if married))
- Particulars of Employment (Occupation, Employer’s Address,
- Name of Employer, Commencement Date of Employment,
- Salary/Wages, Employer’s Telephone)
The following documents needs to be included as part of the social security number application:
- Passport Copy
- Certificate of Naturalization
- Original Birth Certificate
- Baptismal Certificate (if applicable)
- Work Permit (if applicable)
- Marriage Certificate (if applicable)
- Company’s Certificate (if applicable)
